Know your records.

Ask for your medical records, get them reviewed by an AI you choose, then decide what to do next. In three steps, in plain English.

The three steps

  1. Request your records

Write a free request letter (a subject access request under UK GDPR, Article 15) to your GP or hospital.

    Request my records
  2. Review them with an AI

Add your records. You choose the AI. You get a short summary, a dated chronology and points you may want to ask about. You can ask questions too.

    Review my records
  3. Choose what to do next

Write a letter to PALS or the trust’s complaints team, or download or print your report.

How your information is handled

  • Stored on this device only. This website has no server that receives what you type. No accounts, no analytics, no cookies.
  • The AI is your choice. Nothing goes to any AI unless you ask, and for an online AI you see a warning and must confirm first. Read more.
  • You can wipe it all. Use “Delete all my data” at the top or bottom of any page.
  • Not advice. We give general information and templates. We do not say whether anyone was at fault. Terms and “not advice”.

In an emergency call 999. For urgent medical advice call NHS 111. This site is written for England; Scotland, Wales and Northern Ireland have their own NHS complaints routes.

Delete all my data?

This removes everything this site has saved in this browser: your request letter details, the text of your records (if you chose to keep it), your report, your complaint letter and your AI choice. It cannot be undone. Anything you already copied into another website is not affected.
